According to the report, a 2011 Freightliner, driven by 56-year-old Braulio Martinez of Ontario, failed to stop his truck near an unrelated accident, and slammed into a 2012 Dodge Caravan, which had five people inside.
The driver of the Dodge and two other young girls died due to the impact. They were identified as a 57-year-old woman and the two girls were 6 and 10 years old. Their names have not been released.
Two other passengers in the Dodge, identified as another 6-year-old girl, who suffered major injuries and 19-year-old Raylene Valencia, who suffered minor injuries, were transported to a nearby hospital.
